Medical Nutrition Therapy (MNT) is a nutrition-based treatment provided by a licensed, registered dietitian nutritionist. MNT is effective in improving wellness and is an essential component of comprehensive healthcare.As a vital part of the healthcare team, our registered dietitian provides nutrition education and specialized nutrition support to inpatients and outpatients of all ages. Our registered dietitian takes an individualized approach to nutrition and will meet you wherever you are in your nutrition journey to build lifelong healthy habits.

Long Term Care Nutrition Services
All New Horizons Care Center residents are assessed by our registered dietitian upon admission and quarterly, at minimum. If residents are considered high nutrition risk (e.g. malnutrition, wounds, weight loss), monitoring will occur more frequently. The registered dietitian also assists with menu development to ensure that our menus are nutritionally adequate.Inpatient Nutrition Services
Our medical providers and registered dietitian work closely to order diet and nutrition supplement protocols that meet the needs of our patients. Nutrition care needs are identified for hospitalized patients through a nutrition screening process upon admission. Patients identified as at nutrition risk are referred to the registered dietitian who will complete a nutrition assessment and care plan for the patient.Outpatient Nutrition Counseling
Our registered dietitian nutritionist is available for individual consultations to manage, prevent, and learn more about:• Blood pressure

To schedule an outpatient appointment with the registered dietitian, you will first need a referral from your physician.
Before your first outpatient visit with the registered dietitian, please check with your insurance company to see if your plan covers nutrition services (Medical Nutrition Therapy or MNT).
Medicare Part B insurance covers medical nutrition therapy services if you have diabetes or kidney disease, or you have had a kidney transplant in the last 36 months.
If you are on a commercial insurance plan, please verify your coverage for 97802 – MNT, Initial and 97803 – MNT, follow up if being referred for medical nutrition therapy before your first appointment.
We will bill your insurance on file for your visit; however, any charges not covered by the insurance company are your responsibility.
